CBT vs. EMDR vs. EFT: Which Therapy Modality Is Right for You in Calgary?Not all therapy is the same. The modality — the specific approach your therapist uses — is one of the strongest predictors of outcome for your presenting concern. This guide explains the three most commonly requested modalities in Calgary and which conditions each one is best suited to address. The answer to that question narrows your search dramatically and significantly improves the probability of a good outcome. Cognitive Behavioural Therapy (CBT)What It IsCBT is the most extensively researched psychotherapy modality in existence. It is based on the relationship between thoughts, emotions, and behaviours — specifically, the observation that distorted or unhelpful thinking patterns produce negative emotional states and behavioural responses that maintain those states. CBT works by identifying the specific thoughts driving distress, testing them against evidence, and replacing them with more accurate and adaptive alternatives. What It Treats BestCBT has the strongest evidence base for: generalized anxiety disorder, panic disorder, social anxiety, depression, OCD (specifically using ERP, a variant of CBT), phobias, insomnia, health anxiety, and PTSD. It is the recommended first-line treatment by most clinical guidelines for anxiety and depression specifically. What to ExpectCBT is structured and time-limited — most courses run 8 to 20 sessions. Sessions involve identifying specific thoughts, completing worksheets and thought records between appointments, and building skills that generalize beyond the therapy room. It is active and collaborative. You will not be sitting in silence — you will be working. LimitationsCBT addresses cognition and behaviour effectively but does not always fully address the physiological and somatic dimensions of trauma. For clients with complex or developmental trauma, a body-informed approach alongside or instead of CBT often produces better outcomes. Eye Movement Desensitization and Reprocessing (EMDR)What It IsEMDR is a trauma-specific therapy developed by Francine Shapiro in the late 1980s and now endorsed by the World Health Organization and the Canadian Psychological Association as a first-line trauma treatment. It uses bilateral stimulation — typically guided eye movements, but also auditory or tactile stimulation — while the client holds a traumatic memory in mind. The bilateral stimulation appears to facilitate the reprocessing of traumatic memories by allowing the brain to integrate them with proper temporal and contextual encoding, so they are stored as past events rather than present threats. What It Treats BestEMDR has the strongest evidence for: PTSD and complex PTSD, single-incident trauma, developmental and childhood trauma, grief and loss, phobias, and some presentations of anxiety and depression with a traumatic origin. It is particularly effective when talk therapy has been insufficient or when the client has difficulty verbalizing traumatic experiences. What to ExpectEMDR follows a structured eight-phase protocol. The preparation phase ensures sufficient stabilization before processing begins. Processing phases involve targeted recall of traumatic memories alongside bilateral stimulation in a carefully paced, therapist-guided sequence. Clients frequently describe results as faster than expected — significant reduction in trauma symptoms in fewer sessions than CBT produces for the same presentations. LimitationsEMDR requires a trained and certified practitioner. Not all therapists who describe themselves as EMDR-trained have completed full accredited training and supervised practice hours. Emotionally Focused Therapy (EFT)What It IsEFT, developed by Dr. Sue Johnson, is the most evidence-based approach to couples therapy currently available. It is grounded in attachment theory — the understanding that adult romantic relationships function as attachment bonds, and that relationship distress is fundamentally driven by attachment insecurity and the emotional responses it generates. EFT works by identifying the negative interaction cycles that maintain distress, accessing the underlying attachment emotions driving those cycles, and restructuring the couple's interaction around emotional openness and responsiveness. What It Treats BestEFT has the strongest evidence for: relationship distress, emotional disconnection, communication breakdown, trust ruptures, and the relational impacts of trauma, depression, and chronic illness. Research shows that 70 to 75 percent of couples who complete EFT move from distress to recovery, with gains maintained at two-year follow-up. What to ExpectEFT is not skills-based in the same way CBT is. It does not primarily teach communication techniques. It works at the level of emotional experience — helping partners access and express the vulnerable emotions underneath reactive patterns, and helping the other partner receive those emotions with empathy rather than defensiveness. Sessions can be emotionally intense. Progress tends to be clearly felt as well as intellectually understood. LimitationsEFT requires both partners to be willing to engage with emotional vulnerability. If one partner is unwilling to examine their own role in the cycle, the model is harder to apply. EFT also requires a therapist with specific training — it is not interchangeable with general couples counselling. How to Choose
